      <image:caption>Übliche Veränderungen der Struktur der Bandscheibe Während der Degeneration nimmt der Anteil an wichtigen Bausteinen wie Proteoglykanen (die für die Feuchtigkeit in der Bandscheibe sorgen) ab. Dies führt dazu, dass die Bandscheibe weniger Flüssigkeit anziehen kann und an Elastizität verliert. Außerdem wird das Kollagen in der Bandscheibe umgebaut: Das elastische Typ-II-Kollagen nimmt ab, während das festere Typ-I-Kollagen zunimmt, was die Bandscheibe steifer und weniger flexibel macht. (1) Diese Veränderungen sind nicht mit Schmerzen assoziiert und sind normale Veränderungen der Wirbelsäule bzw. Bandscheiben. (2) Verletzungen, Entzündungen und Schmerzursachen Ohne entsprechende Belastung der Bandscheiben bzw. der Wirbelsäule können im äußeren Faserring Risse entstehen, und die Bandscheibe verliert an Stabilität. Diese strukturellen Veränderungen können zu Bandscheibenvorwölbungen (nur ein kleinerer Einriss des äußeren Faserringes) oder Bandscheibenvorfällen (Risse des Faserringes an einer Stelle) führen. (1) Durch die Verletzungen können Entzündungsstoffe in die Bandscheibe eindringen. Diese Stoffe, wie z.B. Zytokine, fördern die Entzündung und den weiteren Abbau der Bandscheibe. Zusätzlich regen sie das Wachstum von Nervenfasern in die normalerweise nervenfreie Bandscheibe an. Diese Nervenfasern können Schmerzsignale aussenden, was zu Rückenschmerzen führen kann. In fortgeschrittenen Stadien der Degeneration kann durch das Vorwölben einer Bandscheibenverletzung auch Druck auf eine Nervenwurzel entstehen, was ausstrahlende Schmerzen verursachen kann. Am häufigsten betroffen ist die Supraspinatus-Sehne, die oberhalb des Schultergelenks verläuft. (2) Aber wie entstehen diese Kalkablagerungen? Viele Patient:innen kommen zu mir in meine Praxis mit einem völlig falschen oder veralteten Bild. Früher wurde die Theorie vertreten, dass sich mit dem Alter einfach die Sehnen der Rotatorenmanschette abnutzen, porös und dünn werden und sich dadurch Kalk in den Mikro-Rissen der Sehnen bildet. Das erklärt allerdings nicht, warum sich Kalkablagerungen auch wieder zurückziehen können und schrumpfen bzw. aufhören, sich auszuweiten – mit höherem Alter sollten die Sehnen je weiter „abnutzen“ und sich immer mehr Kalk ablagern?
